OVERVIEW

The College Park Seniors Program provides supportive assistance and advocacy services to the senior residents of the City of College Park for the purpose of enhancing quality of life in health, social and emotional support.

SERVICES

□  Advocate for Seniors with:
     - Social Security and Medicare  
     - Medical Assistance
     - Health Insurance Plans
     - Prescription Programs including Part D
     - Insurance Benefits
     - Medical Providers   
     - Other Financial and Government       
       Entitlement Programs

□   Information and Referral to other Community and County Services.

□   Free Limited Transportation.  
     - Nearby Medical Appointments.
     - Pre-Scheduled Trips to Grocery Store.
     - Contact Program Staff for Details.

□  Day Trips
     -  8 Trips per Calendar Year

□  Outings for Seniors with Wheelchairs
     -  8 Trips per Calendar Year

□   Annual Health Fair (Fall)

□   College Park Activity Team
     - consists of Resident Seniors and Program Staff who plan five Activities per Year
 
□   Periodic Presentations on Senior Topics in Safety, Wellness and Health.
